Transition to Adulthood 

 Attention all students ages 15 and over: Massachusetts DOE Transition Planning Form 28M/9 is a mandated form for transition planning prior to or at the time of yearly IEP development. For more details: www.doe.mass.edu/sped/links/transition.html 

Easter Seals of Massachusetts Youth Transition Services: High School students with disabilities, their families and educators are given tools to help achieve their vision. The Tempo Young Adult Resource Center
offers supports and services for young adults (ages 17 - 24)  and addresses the urgent need for services for at-risk youth at the critical point of transition into adulthood. Tempo works with youth who have mental health conditions, a lack of family connections, risky behavior, and other issues such as substance abuse and homelessness. Contact the TEMPO Program Director, (508) 879-1424 x 223, 68 Henry Street, Framingham, MA 01702

Substance Abuse Help: Massachusetts General Hospital: Parents: Coaching, Coping, and Communicating Parents: Coaching, Coping, and Communicating is a program designed for parents whose adolescent children are struggling with substance use.   The adolescents do not need to be in treatment. The group is free of charge and is held every Tuesday from 6 pm - 8 pm at the Addiction Recovery Management Service (ARMS) office at 60 Staniford St , Boston. Refreshments are served. College/Scholarships/Internships

Video interviews with successful college students with disabilities offers first hand accounts of their experiences. Modules include activities to help students explore more about themselves, learn what to expect from college, and considerations and tasks to complete when planning for college.

"Advocates in Motion" Social Program: This Massachusetts Down Syndrome Congress program provides fun, inclusive, interactive events and fosters social relationships for young people with Down syndrome, ages 13-22, one Sunday per month in Waltham. Activities offered include fitness and healthy lifestyles, community service experiences, job/career skills, arts, sports course with Outdoor Explorations, Zumba dance lessons, a holiday dance, Compelling Fitness, a musical theatre workshop provided by Access to Theatre, the annual MDSC Buddy Walk, and a Halloween Celebration.
