Transition Assessments

Creating a Post-Secondary Vision Special Needs Workshop Bridgewater MaPlymouth, January 26th, 2019. Transition assessments are an ongoing processes of data collection on an individual’s needs, preferences and interests as they relate to the demands of independent living. In this workshop participants will learn how to navigate:
• Age Appropriate Transition Assessment Toolkit
• Formal and Informal Transition Assessment Models
• How to effectively weave the Assessment findings into the IEP and TPF.
About the Presenter: Mary Brennan is a certified Special Education Teacher and DESE Endorsed Transition Specialist, working in Special Education for 30 years. She recently received her Transition Specialist Endorsement through UMASS Boston, and is a post-secondary transition teacher in Bridgewater. Her students participate in the Inclusive Concurrent Enrollment Initiative, Self Advocacy Leadership Series and Transitions at Bridgewater. She specializes in Person Centered Planning, age appropriate assessments and connecting with adult agencies for student employment opportunities.
